ADULT AND LONG TERM CARE UNIT provides services to people who need assistance to function as independently as possible within their local community.

PROGRAMS
Community Options Program (COP)
is contracted with local providers.  These services include:
         
  Assessments
            Case Planning
            Case Management
            Community Services as an alternative to nursing home placement

Supportive Home Care funds housekeeping services for elderly and disabled

Supplemental Security Income Exceptional Expense Supplement (SSI-E) provides additional income for SSI eligible individuals needing 40 hours of assistance each month to maintain independent living

Adult Family Home  certification for 1-2 bed family homes to care for elderly and disabled

Elder Abuse and Neglect Investigations -- when a person age 60 or older is subjected to physical abuse, financial exploitation, or neglect, including self-neglect, it is elder abuse.  To report elder abuse call (715)395-7457.

Adult Protection Services, including elder abuse and neglect investigations, guardianship, and protective placements petitions and reviews
Case Management includes locating, coordinating, and monitoring services as well as collaborating with other service providers and informal supports needed by individuals and their families
